Employee Life Options Plus (ELOP) Insurance

Guaranteed Issue Voluntary Purchased Permanent

  • Available to most employer groups with 25 or more employees

  • No regulatory requirements - no participation requirements

  • No direct cost to employer

  • All employees are eligible who are working full time and are between the ages of 18 and 72

  • No medical questions or physical are necessary within the guaranteed issue amounts

  • Policy is portable with no changes in rate or coverage

  • Coverage is available for a spouse whether or not the employee applies

  • One health question for spouse

  • Optional Riders include Children’s Term Rider, Accidental Death Benefit, Payor Waiver of Premium, Level Term Rider, and a Catastrophic Loss rider

Employee Accident Option Plus (EAOP)

  • Choice of 24 hour coverage or off the job only coverage

  • Indemnity amounts payable for many triggers such as: Hospitalization, Emergency Room, AD&D, Transportation, Follow-up Care, Burns, lacerations, fractures, dislocations

  • Optional Riders for Hospital confinement due to Sickness, Enhanced Emergency Room Benefit, Wellness Benefit, Enhanced Physician Office/Urgent Care Benefit and Occupational HIV Benefit

  • Guarantee issue on the base plan for eligible employees, spouse and children

  • Policy is portable should the employee leave his/her employer

Employee Critical Illness Plus (ECIP)

  • Pays 100% of the benefit amount for the diagnosis of the many covered conditions, Includes Cancer, Heart Attack, Stroke, End Stage Renal Disease, Major Organ Transplant, Coma, Paralysis, Severe Burns, Loss of Sight, Speech or Hearing, Alzheimer’s Disease, Amyotrophic Lateral Sclerosis (ALS) and Benign Brain Tumor

  • Pays 30% of the benefit amount for the Coronary Artery Bypass, Angioplasty, and Stent Insertion and Carcinoma In Situ

  • Children are covered to age 26 regardless of the material or dependency status - a 25% benefit is automatically included at no additional charge

  • Children have additional coverage for a new diagnosis of childhood conditions - Cerebral Palsy, Cystic Fibrosis, Down Syndrome, Spina Bifida and Cleft Palate/Lip

  • Includes a one-time Skin Cancer benefit of $300

  • Cancer benefit can be carved out to work with other coverages in place

  • No lifetime maximum - includes benefits for recurrences and additional occurrences

  • Coverage has a 30 day waiting period starting from the application date. The 30 day waiting period is not applicable in some states

  • This is a limited benefit plan
